REGISTER YOUR TRIP

As soon as you have received an official invitation from an Assemblies of God World missionary, please register your trip by sending the MAPS Trip Registration form to AGWM. This will allow time for the team application specialist to communicate with your AG missionary for approval. If you are working with an AG missionary, you are REQUIRED to register with AGWM and purchase GTL insurance. Please see information regarding the required GTL insurance below. Only general information is needed at this time.
Your team application specialist will email you a confirmation number after she receives your trip registration form.
The form may be filled out on-line and returned by one of the following methods:

Construction Trips

World Missions requests that all MAPS construction project funds be sent through the MAPS Construction office. Please download the MAPS Construction Projects form and include with your check made payable to MAPS Construction.
Registration Instructions for Adults (anyone aged 18 and over)
Please have each adult team member complete the following registration forms: Assumption of Risk, GTL Insurance Designation, Code of Conduct, Team Member Emergency Contact, and *Background Check Release. Social Security numbers are required to run background checks.
*If your church has current background checks on the team members, you may complete the MAPS TEAM BACKGROUND CHECKS form to verify that background checks have already been run.
If any of your team members have received the AGWM "volunteer card," they will only need to submit a copy of their volunteer card. They do not need to complete the registration forms listed above. The Volunteer Card is not an insurance card.
Registration Instructions for Minors (anyone under the age of 18)

These forms should be completed by parents/guardians for their minor children: Parental Consent/Authorization (BOTH parents need to sign this form even if they are traveling with their minor), Team Member Emergency Contact, and Temporary Guardianship. (The Temporary Guardianship is an additional form needed if a minor is traveling without either parent). All minor forms must be notarized except the Emergency Contact.
Parents are strongly encouraged to travel with their minor.
GTL insurance is required as part of the registration process. GTL insurance coverage is not intended to replace a team member's current coverage but is in addition to their coverage. In case of a medical emergency or accident, the host missionary normally takes care of making the arrangements for medical treatment and/or evacuation with AGWM's assistance. AGWM has a long relationship with GTL due to their high standards and low cost. Therefore, we require all teams to be insured through GTL.
Instructions for Submitting the Registration Forms to AGWM
Write your insurance check payable to AGWM, account number 302959-2 (08) in the memo line. You may also pay with a credit card by calling Eula Bradford (Insurance Specialist) at 417-862-2781 Ext. 3136.

Please mail the registration forms for adults and minors, along with the Insurance List Report and insurance check to:

IMPORTANT: All team forms need to be received by AGWM at least one month prior to departure. Adding team members within two weeks of departure may result in delay of the application process.
